Signup Form for Transport to Low-Cost Spay/Neuter Clinic


Kitten Division transports to Murphy’s Spay Neuter Clinic in Sunrise Beach twice a month and can transport your cat(s) to the clinic. This is a transport service offered by Kitten Division in an effort to help reduce the cat population in our area.

NOTE: Kitten Division is NOT a veterinary clinic, we are a shelter that offers transportation for your cat(s) to the low-cost veterinary clinic.

You may also sign-up in person at Kitten Division, 996 Missouri Avenue, St. Robert during our regular business hours of 2 to 6 pm.

If you are signing up more than one cat, a separate form must be completed for each.

$50 - Spay/Neuter Surgery, Rabies Vaccination, FVRCP Vaccination, Microchip (Owned Cats)


We only have a limited number of space available on our transport to the clinic and since we have had problems in the past with no-shows, we now require pre-payment to guarantee your appointment. Once we have you placed on the clinic date you requested, we will contact you by email with payment information.
By submitting this form I understand this surgery cannot be expected to change any physical or behavioral problems that this pet may have. I also understand that this clinic only offers spaying and neutering for young, healthy animals and that any other medical issues/concerns need to be addressed with my regular veterinarian. I have provided all information to the best of my knowledge. In addition, I understand that The Spay & Neuter Clinic, LLC has the right to decline surgery on this animal if they believe that the animal’s health is at risk due to medical concerns that the animal may have.
By submitting this form, I, the owner for this animal, certify that this animal will have been fasted for at least 8 hours prior to drop off for surgery. I understand that this is crucial to the safety of the animal during surgery. I, the owner, have the authority to execute this consent and authorize Dr. Cheryl Albin to perform the spay/neuter procedure. I understand that during this performance of the foregoing procedure, unforeseen conditions may be revealed that necessitate an extension of the procedure or different procedures than those set forth. Therefore, I hereby consent to and authorize the performance of such actions as are necessary by the veterinarian’s professional judgement. I also authorize the use of appropriate medication, anesthetics, and hospital support personnel, to be employed and used as deemed necessary by the veterinarian. I have been advised as to the nature of the procedures and the risks involved. I realize that results cannot be guaranteed.
